What Is Vaginismus?

Vaginismus is an involuntary tightening of the vaginal muscles that makes penetration, whether during intercourse, a pelvic exam, or even tampon use — painful, difficult, or sometimes impossible.

Some women describe it as feeling like their body is “shutting down” or “hitting a wall” when penetration is attempted. It can be frustrating, emotionally distressing, and can place strain on relationships. But it’s important to know: this condition is real, valid, and treatable.


Types of Vaginismus

Vaginismus can present in different ways, and understanding the type can help in seeking support and treatment.

(a) Primary Vaginismus

Present from the very first attempt at penetration (such as when trying to use a tampon, undergo a medical exam like a Pap smear, or during a first sexual experience).

(b) Secondary Vaginismus

Occurs after a period of pain-free penetration. It can be triggered by childbirth, infections, menopause-related changes, surgery, or past trauma, leading the muscles to tighten in response to pain.

For women, it often leads to

  1. Persistent pain, discomfort, or fear around intimacy
  2. Avoidance of medical checkups or gynaecological care
  3. Emotional strain such as guilt, frustration, or shame
  4. Lower self-esteem and a sense of isolation

For partners, challenges may include:

  1. Confusion and helplessness when intimacy feels impossible
  2. Fear of hurting their partner or being rejected
  3. Emotional distance and reduced physical closeness
  4. Feelings of inadequacy or resentment
